Stellar Home Health’s Home and Community Based Services (HCBS) bring trained, caring direct care professionals into your home to help your loved one build life skills, get dependable help with daily living and stay connected to the community. And when you need a break, our respite care gives you time to rest and recharge, knowing your loved one is in good hands.
We provide three core HCBS services: habilitation, attendant care and respite. Each is personalized to your loved one’s needs, goals and daily routine.
What Are Home and Community Based Services?
Home and Community Based Services (HCBS) are supports that help people with developmental disabilities and other care needs live as independently as possible at home and in their community, rather than in a facility. In Arizona, many HCBS services are available to eligible members through the AHCCCS Division of Developmental Disabilities (DDD). Stellar Home Health also offers in-home support through private pay for families who need help outside of DDD.
Habilitation
Habilitation services help individuals with developmental disabilities learn, build and maintain the skills they need for greater independence. Our direct care professionals work one-on-one with your loved one on goals that matter in everyday life.

Attendant Care
Attendant care provides hands-on help with the activities of daily living, delivered with dignity and respect for your loved one’s independence.

Respite Care

Paying for HCBS: DDD and Private Pay
DDD: Many in-home HCBS services, including habilitation, attendant care and respite, may be covered through the AHCCCS Division of Developmental Disabilities (DDD), depending on eligibility and the services in your plan. Your DDD Support Coordinator can confirm what you are approved for.
Primary insurance: HCBS is usually not covered by primary insurance, because it is often classified as long-term care.
Private pay: We offer private pay options with customizable in-home support, such as personal care and daily living help, so your family can get the help it needs. Contact us to talk through options and scheduling.

How to Get Started With HCBS
- Contact us. Call 480-809-2823, email info@stellarhomehealth.care, or send a message.
- Connect with your Support Coordinator. If you are a DDD member, let your Support Coordinator know you would like Stellar Home Health as your provider. Not with DDD? Ask us about private pay.
- Assessment and planning. We learn about your loved one’s needs, goals and routine and build an individualized plan with you.
- Caregiver match and scheduling. We match your family with a direct care professional and set a schedule that works for you.
- Care begins at home. We check in regularly and adjust the plan as needs change.
